Abstract
최근 치과병사용 진단서를 발급 받으려는 환자가 증가하는 추세이며 이중 대부분이 악 관절 원판변위를 주소로 하고 있다 병사용 진단서의 중요성은 잘 알려진 바와 같이 국가의 안보 및 사회의 안녕과 질서를 유지하며 넓게는 정의 실현에 그 기반을 두고 있다, 근래에 불거지는 원정출산, 해외이민 등의 병역기피의 편법이 사회적 문제 로 대두되는 이때에 나라의 안정을 해치는 도덕적 해이 (moral hazard)를 잠재우는 막중한 책임의식을 가지고 병사용 진단서를 발급하여야 할 것이다. 이 연구의 목적은 최근에 점차 증가되고 있는 관절원판변위를 주소로 병사용 진단서를 교부 받은 환자들의 진단차트와MRI(magnetic resonance imagmg) 검사결과를 ? RDC/TMD (research diagnostic criteria for temporomandibular disorders) 에 기준하여 분류하고 또한 분류된 질환 별로 치료를 시행한 환자의 빈도를 알아봄으로써 향후 불필요한 병사용 진단서의 발급요구를 자제하도록 유도하며, 증상이 심할 경우에는 조속히 치료를 받도록 권고하여 건전한 군복무를 수행할 수 있도록 조언함으로써 건강한 사회가 이룩되도록 일조하는데 있다. 월부터 2003년 8월까지 서울대학교 치과병원 구강내과 구강안변동통 진료실에 악 관절 원판변위를 주소로 내원한 환자 중 MRI 검사를 받은 환자 274명을 대상으로 하였다. 연구방법으로는 환자의 악 관절장애 차트와 MRI사진을 기준으로 하였다. 274 명중 일반 환자는 114명이었고, 병사용 진단서를 발급 받은 환자는 160명 이었다. 일반 환자의 연령은 13세에서 74 세로 평균 연령은 24,43±8.44 세 이었으며, 병사용 진단서를 발급받은 환자의 연령은 l8세에서 30세로 평균연령은 21.14±2.48세 이었다. 환자의 악관절장애 차트는 RDC/TMD 에 의해 임상분류 되었고 MRI사진에 의해 확인되었다. 1. 1994 년 1 월부터 2003년 8월까지 서울대학교 치과병원 구강내과 구강안변 동통진료실에 내원한 환자 중 악 관절 원판변위로 병사용진단서를 발급받은 환자는 94년 3명, 95년 8명, 96년 20, 97년 17명, 98년 3명, 99년 4명, 2000년 17명, 2001년 9명, 2002년 32명, 2003년 47명, 총 160명으로 최근에 증가하는 추세를 보이고 있다. 2. 병사용진단서를 발급받은 환자 160 명을 관절원판변위의 유형별로 분류하면 Ⅱa(정복성 관절원판변위) 환자가 97명 (60.06%), Ⅱc(개구장애가 없는 비정복성 관절원판변위) 환자가 36명 (22.50%) Ⅱb(개구장애가 있는 비정복성 관절원판변위) 환자가 27명 (16.88%) 으로 질환의 심도에 있어 증상이 가벼운 환자가 증상이 심한 환자보다 우세하였다. 3. 같은 기간에 관절원판변위로 진단받은 일반 환자는 114명이었으며 관절원판 변위의 유형별로 분류하면 Ⅱb(개구장애가 있는 비정복성 관절원판변위) 환자가 52명 (45.61%) Ⅱa( 정복성 관절원판변위) 환자가 38 명 (33.33%) Ⅱc(개구장애가 없는 비정복성 관절원판변위) 환자가 2명(21.05%)으로 질환의 심도에 있어 증상이 심한 환자가 증상이 가벼운 환자보다 우세하였다 4. 병사용 진단서를 발급받은 160 명의 환자 중 치료를 받은 환자는 34 명 (21.25%), 치료를 받지 않은 환자는 126 명 (78.75%) 이었으며 , 일반 환자의 경우에는 114 명중 치료를 받은 환자는 67명 (58.77%), 치료를 받지 않은 환자는 47명 (41.23%)으로 병사용진단서를 발급 받은 환자가 일반 환자에 비해 치료받는 비율이 저조하였다. 이상의 결과로 악관절원판변위로 병사용진단서를 발급 시는 RDC/TMD에 의한 임상검사와? MRI 검사로 정확 한 진단을 내려야 하며, 환자에게는 불요불급한 병사용진단서의 발급요구를 자제토록 권고하며 발급 후에도 계속 치료를 받도록 권고하여야 하겠다.
The objective of this study is to identify recent patients who have been issued medical certificates for military service for TMJ disc displacement, review their charts and MRI results according to RDC/TMD, evaluate the number patients who have received follow-up treatments 10 order to reduce the number of unnecessary diagnoses and in cases of patients with symptom, recommend quick treatment therefore inducing a more honest and moral military service to benefit society as a whole From January of 1994 to August of 2003, 274 TMJ disc displacement subjects taken MRI were selected from patients, who visited the orofacial pain clime in the department of Oral Medicine at the Seoul National University Dental Hospital General patients were 114 subjects and patients issued medical certificate for the military service were 160 subjects The mean age of general patients is 24 43±8 44(l3-74), The mean age of patients issued medical certificate for military since is 2114±2,46(l8-30) The method of study was to review their charts according to RDC/TMD and MRI results, and evaluate the number of patients who have received follow-up treatments 1 From January of 1994 to August of 2003, among the patients who visited the or ofacial pain clinic in the department of Oral Medicine at the Seoul University Dental Hospital, 160 patients issued the medical certificates for the military service received diagnoses of TMJ disc displacement In 1994, 1995, 1996, 1997, 1998, 1999, 3000, 2001, 2002, 2003(until August), 3,8,20,17,3,4,17,9,32,47 patients respectively, received diagnoses of TMJ disc displacement and the number of patients increased recently.
2, Evaluating the patients issued the medical certificate for the military service by the type of TMJ disc displacement according to RDC/TMD and MRI, Ⅱa patients disc displacement with reduction), Ⅱc patient (disc displacement without reduction, without limited opening), Ⅱb patients(disc displacement without reduction, with limited opening) were 97 patients(60.06%), 36patients(22.50%), 27 patients(16.88%) respectively, In the level of the seriousness of their condition, less serious patients were more than more serious patients 3 During the same period of time, among 114 general patients with TNJ disc displacement according to RDC/TMD and MRI, Ⅱb patients(disc displacement without reduction, with limited opening), Ⅱa patients(disc displacement with reduction), IIc patients(disc) displacement without reduction, without limited opening) were 52(45 61%), 38(33,33%), 24(21.05%) respectively. In the level of the seriousness of their condition more serious patIents were more than less senous patients
